Rambam in Hilchot Teshuva 1;4 states that if someone commits a sin that warrants karet or is punishable by death, TeShuva and Yom Kippur suspend the punishment, but suffering in this world is what completes the atonement. The source is Tehillim 89;33, where it states: "Then will I visit their transgression with the rod, and their iniquity with the strokes." The stains on the soul block a person from enjoying the holiness of the Torah and mitzvot, and for sins like wasting seed and immorality, a person's body can literally start breaking down at a young age.
